Service & Warranty Request – Implants

One form per implant — you can start a new form immediately after completing one.

This form is for implants only. For abutments, healing caps, instruments, equipment and other products, please use the products module.

You can choose to ship the product(s) yourself or request a return label from MegaGen Benelux. If you request a return label, your implant(s) will be collected by a courier.

You may be asked to provide X-rays or clinical photos. Complete this form on the device where those files are stored.

Completing a form takes approximately 3–5 minutes per implant.
